Important Information from The Clinic

Dear Parents,

In order for me to help your child with their individual medical needs, I would like for you to take a moment to review and fill out the appropriate forms so that I may be prepared to address their health concerns.  These forms, called Action Plans, will need to be filled out if your child has been diagnosed by a physician with any one of the following medical issues (click the link below for the appropriate medical issue to access the form):

* Allergies (food, seasonal, chemical, etc.)
* Asthma
* Bleeding Disorder
* Cancer
* Cardiovascular Issues
* Cystic Fibrosis
* Diabetes
* Inflammatory Bowel Disease
* Migraines
* Seizures
* Sickle Cell
* Vagal Nerve Stimulator
* V-P Shunt

You may fill out the action plan and email it to me or you may print it out and send it with your child on the first day of school.

If your child needs medication to be kept at school, please fill out the Administration of Medication Request form in addition to the action plan. Click the following link to review the Procedures for Administering Medications at School. It has important information about transporting and administering your child's medication.

If your child needs an Epi-Pen at school, please fill out the Administration of Medication Request form and the Hypodermic Injection consent form.  The Hypodermic consent form will need to be signed in the presence of a notary.
